Create a Custom Header or Footer for a Survey in Qualtrics

Overview

In Qualtrics, a custom header or footer can be created for any survey.

Instructions

Step 1. 

From the opened survey, select the “Look & Feel” option on the left navigation pane.

Step 2.

In the new window that displays the custom header/footer message can be entered into the text entry fields by the header and footer sections located on the "General" tab of the left navigation. If specific formatting is required then select the “edit” option underneath either the header or footer text entry fields. If no specific formatting is required then skip to Step 4.

Step 3.

Text or images can be entered into the text box which will show up as the custom header/footer, as well as any formatting that is specified. After the custom header/footer has been entered, the “Save” button must be selected to continue.

Step 4. 

After the custom header and footer have been created and preview of the survey is as desired, the “Save” button will be selected to save the changes.
