Employee Separation

It is the supervisor's responsibility to submit the Employee Separation Form in the following situations:

  • Never worked 
  • Position ended
  • Resignation
  • Retirement
  • Termination 

Supervisors should complete and submit this form as soon as they become aware of an employee's separation. If applicable, any supporting documentation should be attached below.
