Microsoft Forms Registration

Service Description

The purpose of this document is to keep track of any Microsoft Forms developed by College personnel that are used to collect personally identifiable information (PII), sensitive, or protected data. 

Please note that collecting credit card information is strictly prohibited. If you need to collect credit card information you will need to get approval from Financial Operations.

Microsoft does not provide a Forms administration console for ITS staff to see a list of all forms that have been created and who the creator is.  If the Form creator separates from the College, the Form and all data will automatically be deleted by Microsoft 30 days after the creator’s account is deleted.  If the Form creator transfers to a different department, access to the form may need reassigned to another person.  It is up to each department to notify ITS to transfer the form to another person when needed.
